Dr. Kazi is proud to offer the MyCare Program, a leading-edge Chronic Care Management (CCM) service that ensures continuity of care for Medicare patients with Gastrointestinal (GI) related chronic conditions. The goal of CCM is to provide the best possible care by keeping patients out of the hospital and improving their overall quality of life.
We know the pursuit of better health doesn’t stop when you leave the doctor’s office, and we understand that managing your conditions on your own can be tough, that’s why we are offering the telehealth program called the MyCare Program. The program provides an ongoing relationship with a Virtual Care Team at little to no cost (depending on insurance).
Enrollees can Expect to Receive:
- An individualized comprehensive care plan
- Interactive remote communication
- Assistance with medication management
- Coordination of care between providers
MyCare Program patients will have many resources available, including a dedicated MyCare Coordinator. They will regularly check in by phone and email to help keep you on track. To learn more, Watch the MyCare Program Video. Your MyCare Coordinator can also consult with a variety of clinical experts to ensure you are receiving the best care possible. There will be weekly emails that cover topics like education about your specific gastrointestinal condition(s), coaching on improving your diet, exercise tips, and educational content to help keep you motivated — all delivered in a fun, engaging format, such as animated videos or games. Remote Patient Monitoring (RPM) devices, such as a digital scale, may also be provided to track and measure your progress.
